I can see the CV boot disengaged from the wheel on a 2001 Taurus anything I should do?

I have a friend that says it would be a case of just repacking it with grease as long as there are no holes in the boot and reconnecting it, or if there are he says just replace the entire axel, any suggestions or concerns?
